  Signing Up With An Invitation

  Create an Account from an Invitation sent to your email.

  Request an invitation by giving your email address to a staff member at your providers office.

  1. Click on the registration link that is provided in your email, it comes from noreply@followmyhealth.com
  2. Select the “Sign Up and Connect” option
  3. Next is the username and password screen.  By default, your email address will be in the username field.

  If you share an email, or you want to use a different username, you can edit that field.

  1. Next you will need to read and agree to the user terms.
  2. Enter the Invitation Code, which will be either the last 4 of your SS# or your Birth Year.
  3. Click “Agree” to release of information.

  You will then be ready to securely access & manage your personal health record online from any computer, smartphone, or tablet.

   

  Signing Up Without an Invitation

  The following instructions to request connection to FMH without an Invitation sent to your email from our office.

  • Go to followmyhealth.com on a computer or mobile device using Google.  (if using a mobile device, you will be required to download the FMH app in your app store).
  • Press “log in/sign up”
  • Press I need to sign up
  • Enter the required information, please include your phone number and last 4 of your Social Security number for identifying purposes, so there is not a delay in connection.
  • Press “Confirm & Continue”
  • Accept the “Terms of Use”
  • Confirm or Create a Username
  • Create a Password
  • Press “Confirm & Continue”
  • Next request connection.  Search by zip code, provider (doctor’s name), or name of organization (Southern Indiana Community Health Care).
  • Press connect next to your requested organization
  • Press “Next”
  • Sign and request “Access” and Press “Accept”

  Your request will now be Pending Approval.

  At this time your account will be active, but your records will not be connected until we approve. If you do not see your records in the My Health Tab within 48 hours, Please Call 812-723-3944 and ask for Kris. Inform her that you have requested connection for FMH.
